Amedisys Home Health in Phoenix, AZ, is seeking a Physical Therapist for a full-time day position to help patients stay active and independent at home. You will assess, treat, reexamine, and discharge patients within a home health setting, coordinating care with the broader team.

Qualifications

• Current license to practice physical therapy in the state of assignment.
• Graduate from an accredited physical therapy program per CAPTE.
• One year of related PT experience for home health setup; if less, regional leadership approval and mentoring may be required.
• Current CPR certification.

Responsabilités

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations (every 30 days at minimum), modify interventions as needed, and perform discharge planning.
• Other duties as assigned.
