Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from an CAPTE-accredited PT program.
• One year of home health PT experience preferred.
• Current CPR certification.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, reexaminations (every 30 days minimum), modify interventions as needed.
• Perform discharge planning and other duties as assigned.
