Amedisys is seeking a Physical Therapist to deliver high-quality home health care in a full-time days role. You will assess, treat, and educate patients in their homes, focusing on restoration and functional movement with a patient-centered plan of care.

Qualifications

• Current physical therapy license for the assigned state.
• Graduate from a CAPTE-accredited PT program.
• At least 1 year of PT experience in home health; mentoring/approval required if less.
• Current CPR certification.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations every 30 days, modify interventions as needed, and perform discharge planning.
• Other duties as assigned.
