Job summary

Optum, part of the UnitedHealth Group family, is seeking a dedicated Physical Therapist to join our Home Health team in New Jersey. You will assess functional status and mobility, help develop an individualized Plan of Care, implement interventions, and monitor patient progress to reduce pain and improve daily function in patients’ homes.

Qualifications

• Current Physical Therapy licensure in state of practice
• Current CPR certification
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists the physician in the development of the therapy plan of care
• Provides services within the scope of practice as defined by state laws governing physical therapy, in accordance with the plan of care, and in coordination with other health care team members
• Observes and reports the patient's response to treatment and any changes in condition to the supervising nurse and/or physician
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning
• Instructs and informs patient, family, and health care team regarding the plan of care, functional limitations, and progress toward goals
• Visits patients according to Plan of Treatment, completes progress notes per policy
• Completes all patient evaluations and develops the PT plan of care within state guidelines
• Reports outcomes of evaluation, goals, and anticipated projected frequency of care
