Tender Care Home Health & Hospice is hiring a compassionate Physical Therapist for Las Cruces, NM. This field-based role provides in-home therapy to patients, focusing on safety, comfort, and functional improvement.

Full-time and PRN options, with competitive salary and comprehensive benefits as part of a supportive, family-oriented culture. Requires license in NM, 2+ years preferred, CPR, and strong communication skills.

Qualificações

• Graduate of an accredited Physical Therapy program.
• Licensed Physical Therapist in the state of practice (required).
• Minimum of 2 years’ experience preferred (PT/PTA).
• Current CPR certification.

Responsabilidades

• Evaluate patients and assist physicians in developing plans of care.
• Provide skilled PT services including ROM testing and exercise programs.
• Educate patients, caregivers, and team members on therapy protocols.
• Conduct re-evaluations and adjust plans to evolving patient needs.
• Maintain timely and accurate clinical documentation.
• Supervise PTAs and Home Health Aides as required.
• Participate in interdisciplinary team meetings and ongoing education.
• Ensure compliance with agency, HIPAA, and infection control policies.
