Job summary

Tender Care Home Health & Hospice is hiring a Physical Therapist for Las Cruces, NM. This field-based role provides home health PT services, evaluating patients, creating plans of care, and guiding caregivers within a compassionate, patient-centered team.

The ideal candidate is a licensed PT with 2+ years of experience, CPR certified, and strong communication and documentation skills. Travel to patient homes is required; occasional weekends or holidays may apply.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Licensed Physical Therapist in the state of practice (required).
• Minimum of 2 years’ experience preferred (PT/PTA).
• Current CPR certification.
• Strong communication, documentation, and organizational skills.
• Reliable transportation and ability to travel to patient homes.

Responsibilities

• Evaluate patients and assist physicians in developing individualized plans of care.
• Provide skilled physical therapy services, including functional assessments, range of motion testing, and exercise programs.
• Educate patients, caregivers, and team members on therapy protocols.
• Conduct re-evaluations and adjust plans to meet evolving patient needs.
• Maintain timely and accurate clinical documentation.
• Supervise Physical Therapy Assistants and Home Health Aides as required.
• Participate in interdisciplinary team meetings and continuous education.
• Ensure compliance with agency, HIPAA, and infection control policies.
