Job summary

Tender Care Home Health & Hospice is seeking a Physical Therapist Assistant to provide goal‑directed PT services in patients’ homes across Las Cruces, NM. You will work under a licensed PT and report to the Director of Patient Services to ensure quality care.

The role involves patient evaluation, implementing prescribed therapies, coordinating with a care team, and maintaining accurate records. CPR and state licensure are required, with opportunities for growth and a supportive, family‑oriented

Qualifications

• Graduation from a 2-year PTA program approved by the American Physical Therapy Association (APTA).
• Minimum of one year of clinical PT experience.
• Current license to practice physical therapy in the state of practice.
• Current CPR certification.

Responsibilities

• Assist in ongoing evaluation of patients referred to physical therapy.
• Implement treatment programs per physician orders and plan of care.
• Coordinate treatment plans with supervising therapists and other health staff.
• Maintain clinical records and document patient progress within required timelines.
• Administer manual therapy, modalities, and home exercise guidance as directed.
