Tender Care Home Health & Hospice in Las Cruces, NM seeks an Occupational Therapist (OT) for Full-Time or PRN roles. The OT will evaluate patients, develop and modify care plans, provide direct therapy, and educate patients and families to maximize functional independence.

Qualifications

• Graduate of an occupational therapy curriculum accredited by the AMA/ABET and NBCOT.

• Demonstrates good verbal and written communication.
• Possess CPR certification.

Responsibilities

• Evaluates patient’s functional status and home environment to identify barriers.
• Develops and updates the plan of care with measurable goals.
• Instructs patient, family, and other team members in therapy activities and home programs.
• Performs and documents evaluations, plan of care, and progress notes within required timelines.
• Supervises and coordinates with home health aides and OT assistants as needed.
