Tender Care Home Health & Hospice in Las Cruces, NM seeks a Qualified Physical Therapist Assistant to provide individualized, goal-directed care under the supervision of a licensed Physical Therapist. You will participate in evaluations, implement treatment plans, and collaborate with nurses and other therapists.

Qualifications

• Graduated from a 2-year PTA program approved by the American Physical Therapy Association (APTA).
• Minimum of one year of clinical practice as a Physical Therapist Assistant or equivalent experience.
• Licensed to practice physical therapy in the state.
• Current CPR license.

Responsibilities

• Assist in ongoing evaluation of patients referred to physical therapy.
• Implement rehabilitative treatment programs per physician orders and PT plans.
• Coordinate treatment plans with supervising PT and other team members.
• Re-evaluate and modify treatment plans as progress dictates.
• Explain goals and home program to families to ensure carry-over at home.
• Maintain patient records and documentation of progress and changes.
