Job summary

BAYADA Home Health Care is seeking a Physical Therapist (PT) for full-time or per diem home health visits in Fort Wayne, Indiana. The PT will evaluate patients, develop care plans, and coordinate with nursing and family members to promote recovery and independence.

The role requires an Indiana PT license and at least one year of PT experience, plus strong communication and organizational skills. EMR experience is preferred; field work involves travel to client homes.

Qualifications

• Indiana PT license is required.
• Minimum of 1 year PT experience.
• Graduate from an APTA-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, transfers, balance, strength, gait).
• Implement wound care and protective interventions.
• Develop and adjust individualized care plans with exercises and gait training.
• Collaborate with nursing on multidisciplinary care plans.
• Educate clients and families on rehabilitative care and safety.
• Document observations and interventions in EMR tablet.
