Job summary

Qualifications

• Minimum two years of experience in adult physical therapy, home health experience preferred.

Responsibilities

• Perform physical therapy evaluation and treatment based on physician orders.
• Establish measurable goals and develop treatment plans for patients.
• Maintain communication among treatment team, physician, patient, and family.
• Document services provided for proper billing.
• Supervise Physical Therapist Assistants and Aides in patient care.
