Adoration Health in Fort Wayne, IN is seeking a Home Health Physical Therapist to deliver patient-centered PT services in home settings.

You will evaluate and treat patients, establish goals, and coordinate care with the treatment team in a supportive, team-based environment. Eligible candidates must hold a PT license and CPR certification, with preference for those having hospice/community experience.

Competenze

• Bachelor's Degree in Physical Therapy from an accredited program.
• New graduates may be considered in select markets based on program availability.
• At least 2 years of appropriate experience as a physical therapist; community/hospice experience preferred.

Mansioni

• Perform PT evaluations and treatments per physician orders, including adaptive equipment.
• Set measurable goals and develop treatment plans to maximize independence.
• Maintain communication among the treatment team, physician, patient, and family.
• Ensure thorough documentation for proper billing.
• Supervise PT Assistants and PT Aides in patient care in accordance with state practice act.
