Qualifications

Responsibilities

• Make home visits to clients in designated Fort Wayne territory.
• Perform mobility, gait, balance, and other PT assessments.
• Develop and implement individualized care plans with exercises.
• Educate clients and families on rehabilitative care and safety.
• Document observations and interventions in EMR on a tablet.
