BAYADA Home Health Care is seeking an experienced Occupational Therapist for full‑time or per diem Home Health Visits in Fort Wayne, IN. You will work with a multidisciplinary team delivering rehabilitative care and skilled nursing to adults and seniors in the home.

Responsibilities include conducting thorough evaluations, developing intervention plans, assessing the home environment for hazards, educating patients and families, and documenting care using an EMR system.

Qualifikationen

• Current Indiana Occupational Therapist license.
• Minimum 1 year of OT work experience.
• Graduation from an accredited OT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Solid computer skills; EMR experience preferred.

Aufgaben

• Make home visits to clients in designated territories.
• Evaluate patients' functional status and develop individualized OT plans.
• Assess cognitive, psychological and motor aspects to support goals.
• Evaluate home environments for hazards and independence barriers.
• Identify equipment needs and environmental adaptations.
