Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from CAPTE-accredited physical therapy program.
• One year of related physical therapy experience for Home Health roles.
• If less than one year, regional leadership approval and mentoring/residency participation.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions and conduct reexaminations at least every 30 days.
• Modify interventions as needed and perform discharge planning.
• Other duties as assigned.
