Job summary

Adoration Home Health and Hospice in Westminster, MD is seeking a dedicated Home Health Physical Therapist to join our team. This full-time role offers 1:1 patient care, opportunities for growth, on-the-job training, and a supportive environment to advance your career in home health.

Responsibilities include evaluating and treating patients per physician orders, establishing goals, documenting thoroughly, and supervising aides as needed.

Qualifications

• CAPTE-approved PT education required.
• Current Physical Therapist licensure in state and CPR certification required; CPR may be offered upon hire.
• Two years of experience in adult physical therapy; home health experience preferred.
• Excellent observation, verbal and written communication skills; problem solving and basic math skills.

Responsibilities

• Perform PT evaluation and treatment per physician orders and procure adaptive equipment.
• Set measurable goals and develop treatment plans to maximize independence.
• Maintain communication with treatment team, physician, patient and family.
• Document thoroughly to support proper billing; supervise PTAs and PT aides as needed.
