Community Home Health of Maryland – A Division of Medical Services of America, Inc. is seeking a Full-Time Physical Therapist to join our home health team.

As a PT in home health, you will deliver patient-centered care in patients' homes across your assigned territory, following care plans and tracking progress to restore mobility and independence. Qualifications

• Active Physical Therapy license in Maryland.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• At least 1 year of physical therapy experience (home health preferred).

Responsibilities

• Provide in-home physical therapy services to patients across your assigned territory.
• Follow individualized care plans, adjusting treatment as needed.
• Help patients reduce pain, improve mobility, and regain independence.
• Work closely with families, physicians, and other care team members.
• Monitor and document progress to ensure goals are being met.
