Job summary

VitalCaring seeks a Physical Therapist (PT) for home health in Fort Worth, TX. This full-time role offers autonomy in delivering patient-centered care in patients' homes, coordinating with a care team and documenting progress in the EMR.

You will assess mobility, develop plans, supervise PTAs, educate patients and caregivers, and support safe transitions of care. A DPT or Master’s, TX PT license, CPR, and 1–2 years' home health experience are preferred.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands‑on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Identify additional patient needs and collaborate with interdisciplinary teams.
• Document patient care and progress accurately and timely within the EMR.
