Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from CAPTE-accredited PT program.
• One year of related PT experience in home health or equivalent.
• Current CPR certification.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Perform interventions, reexaminations at least every 30 days, adjust plans as needed.
• Complete discharge planning and related duties as assigned.
