VitalCaring is seeking a Home Health Physical Therapist to deliver patient-centered care across Crystal River and surrounding communities. You will assess function, develop individualized plans, and provide hands-on therapy in patients' homes, promoting independence and safety.

You'll collaborate with nurses, physicians, and therapists, supervise PTAs where allowed, document progress in the EMR, and participate in team meetings.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master's).
• Current PT license in the state of practice.
• Minimum 1–2 years of experience (home health preferred).
• Current CPR certification and valid driver's license.

• Develop and implement individualized care plans with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety.
• Monitor progress and adjust plans; support discharge planning.
• Document care and progress in the EMR; participate in case conferences.

Service Area: This position will serve patients throughout the Crystal River Home Health branch service area, including Crystal River and surrounding communities within the branch's designated coverage area. The position involves providing physical therapy services to patients in their homes throughout the service area.

What You'll Do

Deliver Patient-Centered Care

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential
• Develop and implement individualized care plans in collaboration with physicians and the care team
• Provide hands-on therapy to improve mobility, strength, balance, and independence
• Educate patients and caregivers on treatment plans, exercises, and safety strategies

Drive Outcomes & Recovery

• Monitor patient progress and adjust treatment plans as needed
• Support safe transitions of care and contribute to discharge planning
• Identify additional patient needs and collaborate with interdisciplinary teams
• Partner closely with nurses, therapists, and physicians to ensure coordinated care
• Document patient care and progress accurately and timely within the EMR
• Participate in case conferences and team meetings

Lead & Support

• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
• Provide guidance and education to caregivers and support staff

What You Bring

• Graduate of an accredited Physical Therapy program (DPT or Master's)
• Current PT license in the state of practice
• Minimum 1-2 years of experience (home health experience preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certification
• Valid driver's license, reliable transportation, and auto insurance
