Job summary

Harmonyhomehealth seeks a licensed physical therapist to join our home health team, providing patient-centered care in patients' homes. You will assess patients, develop plans of care, and deliver interventions to maximize function in everyday settings.

Responsibilities include home safety evaluations, patient/family education, and EMR documentation, with collaboration across a diverse care team. A valid PT license and CPR/BLS are required.

Qualifications

• Active State Physical Therapist License
• Current CPR with BLS
• Valid driver’s license, auto insurance, and vehicle
• Knowledge of Federal, State, and local home health laws/regulations
• Great organization skills with minimal supervision
• Capacity to supervise others
• Ability to remain calm in emergencies
• Proficient EMR documentation

Responsibilities

• Perform physical and functional patient assessments
• Develop a plan of care with patient goals
• Provide evidenced-based direct intervention in patient homes
• Perform Home Safety Evaluations
• Communicate effectively with diverse patients and co-workers
• Educate patients and families in home programs and equipment use
• Submit required documentation in a timely manner
