- Active Physical Therapist license in the state of practice.
- Minimum of 1 year of healthcare experience.
- Valid driver's license and reliable transportation.
- Strong time management and self-scheduling skills.
- Comfortable using tablets, smartphones, and EMR systems.
- Commitment to meeting documentation deadlines.
- Compassionate, patient-centered approach to care.
Responsibilities
- Deliver high-quality physical therapy care in patients' homes.
- Develop, implement, and manage individualized care plans.
- Collaborate with a multidisciplinary care team.
- Maintain timely and accurate documentation.
- Travel locally to patient residences (~20% of your day)
