Job summary

Harmony Home Health & Hospice is seeking a Home Health Physical Therapist Assistant to provide physician-prescribed PT in patients' homes under a PT plan of care. You will perform evidence-based interventions, communicate with diverse patients and coworkers, and document promptly.

The role requires an active PTA license, CPR with BLS, a valid driver's license with insurance, and EMR experience. Strong organization and ability to work with minimal supervision are essential.

Qualifications

• Active State Physical Therapist Assistant License.
• Current CPR with BLS.
• Valid driver's license with state-required auto insurance and an operable vehicle.
• Understanding of Federal, State, and local home health laws and guidelines.
• Strong organization and ability to work without direct supervision.
• Ability to stay calm in emergencies or crises.
• Computer literacy and EMR documentation experience.

Responsibilities

• Provide physician-prescribed PT under a plan of care.
• Deliver evidence-based PT interventions in patients' homes.
• Communicate effectively with diverse patients and coworkers.
• Work under supervision of a PT and report changes promptly.
• Assist with education for patients and families as delegated by the PT.
• Submit required documentation in a timely manner.
