Job summary

Harmony Home Health & Hospice in Utah seeks a Home Health Physical Therapist Assistant to join our care team, delivering physician-prescribed PT in patients' homes under a PT plan of care. You’ll support patients with diverse diagnoses and benefit from a flexible schedule and strong collaboration with the care team.

Required are an active PTA license and CPR/BLS. A valid driver’s license and auto insurance are needed to visit patients.

Qualifications

• Active PTA license required
• CPR with BLS current
• Valid driver’s license and auto insurance as needed
• Experience documenting in EMR systems preferred

Responsibilities

• Provide physician-prescribed physical therapy under a plan of care established by the PT
• Deliver evidence-based direct interventions with patients in their home setting
• Communicate effectively with diverse patients and co-workers
• Work under supervision of a PT and relay patient status or changes promptly
• Submit required documentation in a timely manner
