Job summary

Nursing & Rehab At Home is seeking a licensed Physical Therapist to provide home-based physical therapy services under a physician's plan of care. The role includes coordinating patient care, completing assessments, and contributing to discharge planning in a home health setting.

The candidate will oversee PT assistants and educate family members, with responsibilities spanning case management and progress reporting to ensure goals are met.

Responsibilities

• Provides physical therapy services per plan of care and state laws.
• Initiates therapy programs and educates patients and caregivers.
• Completes OASIS assessments as assigned.
• Acts as Case Manager for assigned caseload.
• Identifies needs for other home health services and refers as needed.
• Prepares and submits clinical progress summaries and discharge plans.
• Supervises PT assistants per policy and state regulations.
• Instructs and evaluates home health aides every two weeks when therapy is sole skilled service.
