Qualifications

• Licensed Physical Therapist in the state of Illinois.

• Valid driver’s license and ability to drive to patient locations ~20% of time.
• Proficient with tablets, smartphones, and EMR software.

Responsibilities

• Provide home health physical therapy per patient care plans and physician orders.
• Collaborate with care team to manage and adjust patient treatment plans.
• Document patient progress in the EMR and meet charting deadlines.
