Job summary

Graham Healthcare Group is seeking a Home Health Physical Therapist in Skokie, Illinois. This full-time role offers 40 hours per week with a typical 8-hour shift and is eligible for substantial compensation in the $80k-$120k per year range, plus bonuses and a comprehensive rewards package as part of Residential Home Health and Hospice services.

Clinicians will collaborate to manage and deliver patient care plans for patients across Chicago, Lincolnwood and surrounding areas.

Qualifications

• Bachelor degree or Master degree in Physical Therapy from an accredited college required
• Licensed Physical Therapist issued by the state in which you will be working
• Minimum of one year healthcare experience
• Current drivers license and ability to spend ~20% of your day driving to/from patient locations
• A commitment to consistently meet critical deadlines for charting
• The skills needed to self-manage your time and schedule
• Demonstrated experience with tablets, mobile phones and EMR software
