Job summary

Graham Healthcare Group is seeking a Home Health Physical Therapist for a role in Justice, Illinois. The position is full-time (40 hours per week) with an 8-hour shift, providing patient care in home health settings and collaborating with a dedicated care team.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited college required.
• Licensed Physical Therapist issued by the state in which you will be working.
• Minimum of one year healthcare experience.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
• The skills needed to self-manage your time and schedule.
• Demonstrated experience with tablets, mobile phones and EMR software.

Responsibilities

• Provide management and delivery of patient care plans.
• Collaborate with care team to deliver patient care in home health settings.
• Support patients across multiple service areas with timely charting and documentation.
