Graham Healthcare Group is seeking a Home Health Physical Therapist to support patients in Lombard and surrounding areas. The role is full-time, 40 hours per week, with an 8-hour shift structure and a staff employment type.

Eligible for a strong compensation package with bonuses and extensive benefits. Qualified candidates hold a PT license in the state, with a minimum of one year healthcare experience and ability to drive to patient locations.

Qualifications

• Bachelor’s or Master’s in Physical Therapy from an accredited college.
• Licensed Physical Therapist issued by the state where you will work.
• Minimum of one year healthcare experience.
• Current driver’s license and ability to drive ~20% of the day.

Responsibilities

• Collaborate with the care team to manage and deliver patient care plans.
• Provide therapy services in home health settings in Lombard and surrounding areas.
• Maintain charting and meet deadlines for documentation.
• Engage in patient education and family communication.
