Job summary

Graham Healthcare Group is seeking a Home Health Physical Therapist for a full-time role in Romeoville, Illinois. The position requires a Doctoral degree in Physical Therapy, a valid state license, and at least one year in healthcare.

Expect 40 hours per week with an 8-hour shift. This role includes a $10,000 first-year bonus and a $5,000 sign-on bonus, plus a comprehensive rewards package and benefits. You will travel to patient locations around Romeoville, Crest Hill and nearby areas.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited college.
• Licensed Physical Therapist issued by the state in which you will be working.
• Minimum of one year healthcare experience.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
• A commitment to consistently meet critical deadlines for charting.
• Demonstrated experience with tablets, mobile phones and EMR software.

Responsibilities

• Manage and deliver patient care plans in home health settings.
• Collaborate with care team to ensure coordinated patient care.
• Maintain timely, accurate charting and EMR entries.
