Job summary

Optum is recruiting a Physical Therapist for Home Health to assess and treat patients in their homes, focusing on functional status, mobility, endurance, and pain. You will develop and follow a plan of care, document progress, and coordinate with physicians and the care team to improve patient well-being.

Qualified candidates have current PT licensure in their state, a valid driver's license with reliable transportation, and CPR certification (preferred within 90 days of hire).

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists the physician in developing the therapy plan of care.
• Provides services within the scope of practice as defined by state laws governing PT practice, per plan of care, in coordination with the health care team.
• Observes and reports patient responses to treatment and changes in condition to the supervising nurse and/or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning per standards of practice.
• Informs patient, family/caregivers and health care team about plan of care and progress toward goals.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Completes all patient evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
