Adoration Health in Douglas, GA is seeking a Home Health Physical Therapist to provide 1:1 patient care, develop treatment plans, and collaborate with the care team in a supportive home health setting.

The role requires CAPTE-approved PT education, current GA licensure, CPR certification, and strong communication skills. On-site schedule is Monday–Friday, with a potential sign-on bonus or relocation assistance for external candidates.

Qualificações

• Graduate of a Physical Therapist Education program approved by CAPTE.
• Current Physical Therapist licensure in State and CPR certification.
• Two years of experience in adult physical therapy; home health experience preferred.
• Excellent observation, verbal and written communication skills; problem solving.

Responsabilidades

• Perform PT evaluation and treatment per physician orders and procure adaptive equipment.
• Establish measurable goals and develop treatment plans to maximize independence.
• Maintain communication among treatment team, physician, patient, and family.
• Maintain thorough documentation for proper billing.
• Supervise PTAs and PT Aides in direct patient care as applicable.
