Job summary

PARS Therapy in Crosby, TX is seeking a compassionate Physical Therapist to join our home health team. This onsite role focuses on evaluating functional abilities, creating individualized plans, and guiding patients toward independence.

You will work with physicians, nurses, and therapists to deliver evidence-based care, educate families, and adapt approaches to diverse patient needs in patients' homes.

Qualifications

• Active Physical Therapist license in Texas.
• Professional liability insurance coverage.
• Graduate degree or higher in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Strong ability to adapt care approaches to patient needs.
• Up-to-date knowledge of evidence-based treatment techniques.
• Excellent communication skills and professional demeanor.

Responsibilities

• Evaluate patients’ physical abilities in a home health setting.
• Develop and implement individualized treatment plans to restore function.
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs and safety.
• Monitor progress and adjust plans to achieve outcomes.
• Document evaluations, sessions, and progress notes digitally.
• Collaborate with physicians, nurses, and occupational therapists.
• Conduct home safety assessments and reduce fall risk.
• Deliver compassionate care to promote independence.
