Job summary

PARS Therapy is seeking a compassionate Physical Therapist to join our home health team in Cleburne, TX. You will evaluate patients, develop individualized plans, and deliver evidence-based interventions in patients’ homes.

This onsite role offers a supportive team environment, flexible scheduling options, and opportunities for professional growth while helping patients regain movement and independence.

Qualifications

• Active Physical Therapist license in TX.
• Professional liability insurance coverage for TX PT license.
• Graduate degree or higher in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Excellent communication skills with patients and caregivers.

Responsibilities

• Evaluate patients in a home health setting and determine functional limitations.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs and safety techniques.
• Monitor progress and adjust plans to achieve optimal outcomes.
• Document evaluations, sessions, and progress notes using digital platforms.
• Collaborate with physicians, nurses, and other therapists as part of an interdisciplinary team.
• Conduct home safety assessments and recommend fall-prevention strategies.
• Deliver compassionate care and empower patients toward greater independence.
