Job summary

PARS Therapy in Conroe, TX is seeking a compassionate Physical Therapist to join our dedicated home health team. This role offers meaningful clinical work delivering patient-centered care in patients' homes.

Responsibilities include evaluating function, developing and implementing treatment plans, gait and balance training, patient and caregiver education, progress documentation, and collaboration with physicians and nurses to promote safe, independent movement.

Qualifications

• Active TX PT license required.
• Graduate degree or higher in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in home health.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs and safety.
• Monitor progress and adjust plans to achieve outcomes.
• Document evaluations, treatment sessions, and progress notes digitally.
• Collaborate with physicians, nurses, and other therapists.
• Conduct home safety assessments and reduce falls risk.
• Deliver compassionate, patient-centered care.
