Job summary

ExcelaCare Home Health in Big Spring, TX is seeking a Physical Therapist for a full-time role. You will deliver skilled PT services in the home setting, following a patient-specific plan of care and AP TA guidelines to improve function and independence for clients.

You will design and implement treatment plans, supervise PTA and aides, collaborate with the health care team, and ensure accurate documentation and timely communication with families.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Two years of PT experience in the last five years.
• Current licensure and CPR certification.
• Valid driver’s license and proof of auto liability insurance.

Responsibilities

• Provide direct patient care per the plan of care.
• Design and implement a Plan of Care based on assessment.
• Evaluate outcomes and discharge planning.
• Supervise PTA, aides, and students.
