Job summary

ExcelaCare Home Health in Plainview is seeking a Physical Therapist for full-time work. You will provide direct patient care, develop and implement a Plan of Care, and collaborate with PTA, aides, and other professionals to improve patient outcomes.

The position requires graduation from an accredited PT program, state licensure, CPR, a valid driver's license, and at least two years of experience in the last five years. Travel to patients' homes is required.

Responsibilities

• Conducts therapy practice within defined standards of care according to APTA guidelines and practice act.
• Designs and implements a Plan of Care for the patient and significant others based upon a thorough assessment.
• Provides direct patient care, evaluates/reassesses patient’s functional needs to ensure optimal outcomes.
• Instructs patients, families, and caregivers in home exercise programs; recommends equipment as needed.
• Develops coordination and cooperation among other health care disciplines to provide quality patient care.
• Completes and submits documentation timely and accurately according to policy.
