Job summary

ExcelaCare Home Health in Hobbs, NM is seeking a licensed Physical Therapist for a full-time, per-visit role delivering in-home care. You will assess impairments, design a Plan of Care, and collaborate with PTA, aides, and other clinicians to ensure quality outcomes for patients.

Responsibilities include evaluating progress, teaching home exercise programs, coordinating with families, and timely documentation.

Qualifications

• Education: Graduate of an accredited Physical Therapy program.
• Experience: Two (2) years of experience as a physical therapist in the last five (5) years.
• License / Certification: Current licensure; CPR certification; current driver’s license and proof of auto liability insurance

Responsibilities

• Provides direct patient care and reassesses patient’s functional needs.
• Designs and implements a Plan of Care based on assessment.
• Instructs patients, families, and caregivers in home exercise programs; recommends equipment as needed.
• Evaluates outcomes and participates in discharge planning.
• Maintains timely and accurate documentation per policy.
• Provides direction to PTA, aides, and students; participates in on-call rotation.
• Coordinates care with other health disciplines to ensure quality patient care.
