Job summary

ExcelaCare Home Health - Plainview, TX seeks a full-time Physical Therapist to evaluate patients, design plans of care, and deliver in-home therapy with limited supervision.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Two years of experience as a physical therapist in the last five years.
• Current licensure; CPR certification; valid driver’s license and proof of auto liability insurance

Responsibilities

• Designs and implements a Plan of Care based on assessment.
• Provides direct patient care and reevaluates functional needs.
• Supervises PTA, aides, and students.
• Communicates with patients, families, and care team.
• Develops home exercise programs and discharge planning.
