Job summary

Advanced Nursing Concepts provides one-on-one home health care to patients in their homes. We support clinicians with mentorship, clinical guidance, and flexible scheduling, ensuring independence in home health without isolation.

Qualifications

• Graduate of an accredited school of nursing.
• 1–2 years of recent acute care experience in an institutional setting.
• Current licensure in state and CPR certification.
• Bachelor's degree preferred, with 1 year of home health care experience.
• Excellent observation, verbal and written communication skills, problem solving skills; nursing skills per competency checklist.

Responsibilities

• Assess each new patient's condition, history, and home care needs.
• Build the care plan with the patient and family.
• Update the care plan as the patient's status changes.
• Administer medications and treatments as prescribed.
• Teach patients and families how to manage care at home.
• Coordinate with physicians and send updates at least every 60 days.
• Coordinate with community health partners involved in the patient's care.
• Plan and carry out discharge when the patient is ready.
• Arrange any equipment or services the patient needs.
• Check in on home health aide care every two weeks.
