Job summary

Vivie is seeking a licensed Physical Therapist in Minnesota for a PRN, hourly, non-exempt position. You will assess clients, create individualized treatment plans, document care in Vivie’s EMR, and coordinate with families, physicians, and the inter-disciplinary team.

You’ll travel to client homes within designated regions, maintain records, and support follow-up programs to optimize outcomes. Strong communication and collaboration are essential, with CPR certification obtained within 6 months.

Qualifications

• Current Physical Therapist license in Minnesota.

• Proficient in English for effective communication with staff, clients, and visitors.
• Successful completion of a state-mandated background check.
• Able to obtain CPR certification within 6 months of hire.

Responsibilities

• Deliver Direct Client Care — conduct evaluations and develop personalized treatment plans with goals.
• Maintain Accurate Client Records — document services using Vivie’s EMR system.
• Facilitate Follow‑Up Programs — oversee continuation programs for long-term success.
• Collaborate and Communicate Effectively — work with clients, families, physicians, and the team.
• Ensure Regulatory Compliance — complete supervisory visits with PTAs and RA/CNAs per guidelines.
• Other Duties as Assigned — include additional tasks as needed.

Pay range

$41.04/hour to $52.00/hour (hourly non-exempt) based on qualifications, experience, and location.

As a Physical Therapist, You Will

• Maintain Accurate Client Records - Document services accurately and timely using Vivie’s EMR system, ensuring compliance with all payer sources and authorization requirements.
• Facilitate Follow‑Up Programs - Recommend and oversee continuation programs to support long‑term client success and optimize discharge outcomes.
• Collaborate and Communicate Effectively - Work closely with clients, families, physicians, and the interdisciplinary team to provide care.
• Ensure Regulatory Compliance - Complete and document supervisory visits with PTAs and RA/CNAs in accordance with state and regulatory guidelines.
• Other Duties as Assigned - The duties and responsibilities listed above are representative of nature and level of work assigned and not necessarily all inclusive.

This Job Also Requires
