Job summary

Caringhand Home Health Solutions, Inc, a home health provider in California, seeks a Physical Therapist for a field-based, per diem role requiring travel to patients’ homes. Compensation is $160-$180 per visit and a flexible schedule is offered.

Responsibilities include evaluating functional status, delivering therapy per plan of care, educating patients, and coordinating with the care team. A CA license, CPR, and 2+ years of PT experience are required.

Qualifications

• Must have CA Physical Therapy license.
• 2+ years PT experience; home health background is a plus.
• Current CPR certification (AHA).
• Valid driver's license and reliable, insured vehicle.

Responsibilities

• Evaluate patients' functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver PT services per physician's plan of care and California PT Act.
• Educate patients and families on exercises, equipment use, and recovery goals.
• Collaborate with physicians and care team; report changes and participate in conferences.
• Supervise PTAs and oversee Home Health Aide care when PT is sole skilled service.
• Manage documentation including progress notes and discharge planning.
• Identify needs for additional home health services and coordinate referrals.
