Job summary

Premier Home Health Providers Inc. is seeking a Physical Therapist to deliver skilled services in patients' homes in California. As part of our compassionate team, you will evaluate function, develop treatment plans, and coordinate with physicians to support recovery and mobility.

This per diem role offers a rate of $160-$180 per visit, with the potential to convert to full-time based on performance. A valid California PT license, CPR certification, and reliable transportation are required.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Current CPR certification with AHA.
• Valid driver’s license and reliable, insured vehicle.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act.
• Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
• Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.
