Qualifikationen

• Licensed Physical Therapist authorized to practice in Colorado.
• Minimum 2 years' home health experience.
• CPR certification.
• Valid driver’s license and reliable transportation.

• Develop and update individualized home health care plans.
• Provide physical therapy services per physician’s plan of care.
• Assess functional status, mobility, and home environment.
• Educate patients and families on therapy techniques and equipment.
• Maintain accurate documentation and communicate progress to physicians.
