Job summary

The Pennant Group, through PASCO/SW Home Health in Grand Junction, CO, seeks a licensed Physical Therapist to administer home-based care. You will design individualized plans, deliver hands-on therapy per physician orders, and collaborate with a supportive team while maintaining scheduling flexibility to fit your lifestyle.

• Must be licensed as a Physical Therapist in Colorado.
• Graduation from a CAPTE-accredited program and passing the state licensure exam.
• Minimum 2 years of experience; home health preferred.
• Current CPR certification and valid driver’s license.

Responsibilities

• Develop and update individualized care plans.
• Provide physical therapy services per physician’s plan of care.
• Assess functional status, mobility, and home environment.
• Educate patients and families on therapy techniques and equipment.
• Maintain accurate documentation and communicate progress to physicians.

OB SUMMARY A Registered Physical Therapist (PT) administers physical therapy to patients in their place of residence and is performed in accordance with physician orders and IDG plan of care under the direction and supervision of the Director/Manager of Patient Services. Grand Junction salary: per visit model $ 95,000.00 - $130,000.00
