The Pennant Group, Inc. is hiring a Registered Physical Therapist for Pasco SW Home Health serving Cortez and surrounding Colorado communities. You’ll design individualized plans of care, deliver hands-on therapy, and coordinate with physicians under supervision to support patients at home.

Qualifications

• Licensed Physical Therapist in the State of Colorado.
• CAPTE-accredited program graduate; passed state exam.
• Minimum 2 years’ home health experience preferred.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Develop and update individualized care plans.
• Provide physical therapy services per physician’s plan of care.
• Assess functional status, mobility, and home environment.
• Educate patients and families on therapy techniques and equipment.
• Maintain documentation and communicate progress to physicians.
