Job summary

Star Light Home Health in Colorado seeks a dedicated Physical Therapist (PT) to join our Medicare Certified Home Health team. Deliver one-on-one rehabilitative care in patients' homes, with autonomy and a focus on outcomes.

Responsibilities include evaluations, treatment planning, gait training, balance, pain management, and completing OASIS and documentation per Medicare guidelines. Travel to Grand Junction area with a patient-centered approach and collaborative care with nurses and physicians.

Qualifications

• Active Colorado Physical Therapist license in good standing.
• Graduate of an accredited Physical Therapy program (DPT or equivalent).
• Experience in home health, geriatrics, or Medicare-certified settings preferred.
• Familiarity with OASIS documentation and Medicare home health regulations.
• Valid driver's license, reliable transportation, and ability to travel to patient homes in the Grand Junction area.
• Strong communication, time management, and critical thinking skills.
• Compassionate, self-motivated, and comfortable working independently in the field.

Responsibilities

• Conduct comprehensive physical therapy evaluations and develop individualized treatment plans for home health patients.
• Deliver skilled PT services including therapeutic exercise, gait training, balance and fall prevention, and pain management.
• Monitor and document patient progress in compliance with Medicare home health guidelines and agency policies.
• Collaborate with the interdisciplinary care team including nurses, occupational therapists, and physicians.
• Educate patients and caregivers on home exercise programs, safety techniques, and functional independence strategies.
• Complete OASIS assessments and maintain accurate, timely clinical documentation.
• Participate in care coordination and discharge planning to ensure continuity of care.
