Job summary

The Pennant Group, through PASCO/SW Home Health in Grand Junction, CO, seeks a licensed Physical Therapist to administer home-based care. You will design individualized plans, deliver hands-on therapy per physician orders, and collaborate with a supportive team while maintaining scheduling flexibility to fit your lifestyle.

Qualified candidates are CO-licensed PTs with CAPTE-accredited education, state exam, CPR, and a minimum of 2 years’ home-health experience; valid driver's license and

Qualifications

Must be licensed as a Physical Therapist in Colorado.
Graduation from a CAPTE-accredited program and passing the state licensure exam.
Minimum 2 years of experience; home health preferred.
Current CPR certification and valid driver’s license.

Responsibilities

Develop and update individualized care plans.
Provide physical therapy services per physician’s plan of care.
Assess functional status, mobility, and home environment.
Educate patients and families on therapy techniques and equipment.
Maintain accurate documentation and communicate progress to physicians.
