Job summary

VitalCaring Group is seeking a Home Health Physical Therapist to help patients regain mobility in their homes. You’ll assess functional status, develop individualized care plans, provide hands-on therapy, and educate patients and families. This role emphasizes autonomy, collaboration with nurses and physicians, and timely EMR documentation.

You’ll supervise PTAs, participate in case conferences, and contribute to discharge planning, ensuring safe transitions and high-quality care.

Qualifications

• Graduate of an accredited Physical Therapy program with licensure.
• Minimum 1–2 years home health experience preferred.
• Current CPR certification and state license.

Responsibilities

• Evaluate patients’ functional status, mobility, and rehabilitation potential.
• Develop and implement individualized plans of care with physicians.
• Provide hands-on therapy to improve mobility and strength.
• Educate patients and caregivers on treatment plans and safety.
• Monitor progress, adjust plans, and document in EMR.
• Supervise PTAs and participate in discharge planning.
