Job summary

Advanced Health Care in Lewiston, Idaho is seeking a licensed Home Health Physical Therapist to deliver patient-centered therapy in home settings. You will evaluate patients, create individualized treatment plans, and implement interventions to improve mobility and daily functioning.

Collaborating with physicians and the care team, you will educate families, document progress, and participate in care planning. This is a full-time, weekday position within a home health program.

Qualifications

• Valid Physical Therapy license in the state where work is performed.
• 2+ years of physical therapy experience, preferably in home health or similar setting.
• Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, caregivers, and healthcare professionals.
• Ability to build rapport with patients through a compassionate and caring demeanor.

Responsibilities

• Conduct thorough evaluations of patients’ physical abilities and needs to develop individualized therapy plans.
• Develop and implement treatment programs to improve mobility, strength, flexibility, and functional performance.
• Educate patients and families on exercises and home programs to support recovery and daily living activities.
• Monitor and document patient progress and communicate with supervising physicians and the healthcare team.
